Certain paperwork is necessary at your initial visit. In order to reduce your registration time, we have made these forms available by following the simple steps below. You can fill them out in the comfort of your home and bring them with you to your appointment.

 It's a very simple process:

2. Print the form and then fill it out by hand.
Sign where appropriate and bring to our office on your first visit.
