Dear Parents:
 

 

 

I would like to let you know that I have recently accepted an appointment to the teaching faculty in the Department of Pediatrics, Division of Child Neurology at the University of South Carolina School of Medicine in Columbia, SC. My scheduled start date in South Carolina is early June. My last clinic date here will be Thursday, April 30.

After living and practicing here for almost 20 years the decision to accept this offer was difficult. However, the opportunity to teach young physicians and assist with the development of a strong child neurology service at the new Palmetto Health Children’s Hospital in Columbia was impossible to turn down.

 

 

 


The excitement of this new challenge for me is tempered by the stress this news may cause you. Your child’s health and comfort is extremely important to me and I will do all I can to assist you with transition of your child’s neurologic care to other available resources. Your primary care physician and the child neurologists in Tulsa and Oklahoma City have been sent letters briefing them on this situation. Drs. Steve Miller and David Siegler, the two board certified child neurologists in Tulsa have graciously agreed to assist with my patients who require ongoing care after I leave.

Although it would be nice to meet with each of you in the office before I go, I realize this may not be necessary or practical for all patients. For those who need or desire an appointment with me before April 30, the enclosed information sheet will assist you. It also provides important information regarding transfer of your child’s medical records.

My transition appointment with you in the coming weeks will allow us to review progress and discuss recommendations for your child. It is important that you have secured follow up regarding care and prescription refills prior to April 30 since I will not be available after that date. Requests after April 30 regarding medical care or refills will be referred to your child’s PCP on record.

Finally, I want you to know that working with you and getting to know your child has been a great joy to me. Your trust in my care has been an honor and privilege that I will always treasure. I will miss you all greatly.
