Instructions for transferring records to your new provider:

1. Contact one of the available resources that Dr. Morgan recommended to request a new patient appointment.


2. Your new provider’s office will inform you of necessary steps in establishing care. This may include requiring your PCP to initiate the appointment request.


3. Our office will fax your child’s medical records to your new provider once their office contacts us with appointment information.

Instructions for transferring records to your new provider:

Our physical office will close APRIL 30. Dr. Morgan will not be available for medical questions, recommendations, completion of forms/letters or prescription refills after APRIL 30. Parents will need to contact their child’s new neurology provider or PCP.

Requests for transfer of records received by mail, phone or fax will continue to be processed promptly by our office on an as needed basis.
